Thank you for your interest in Rocket City Pediatrics! We are so grateful for the support we’ve received from the community. We’ve now made it easier than ever to get started as a new patient with us, here are the steps:
Submit the New Patient Inquiry Forms at the button to the right →
After you have submitted the appropriate forms, a member of our team will request medical records from your previous pediatrician. Under HIPAA, providers must respond within 30 days of receipt of a patient’s request for access to records. However, this process can take up to an additional 30 days if the provider requests an extension. This process can be expedited if you contact your previous provider and ask for records to be faxed to us.
After we have received your medical records from the previous provider, we will call you to schedule your first visit as a new patient with us!

If you are trying to schedule for your newborn, please call the office directly! We love to see newborns within 3-5 days of life to get a comprehensive overview of their health from birth.
